I, parent or legal guardian of the above-named child, authorize New Life Iglesia de Dios Pentecostal M.I. to take and use photographs, videos, or other images in which my child appears during participation in Vacation Bible School.
I understand these images may be used for informational, promotional, or commemorative purposes on social media, websites, presentations, newsletters, and printed materials related to the church.
I declare that this authorization is voluntary and that no compensation will be required for the use of such images.
I, parent or legal guardian of the above-mentioned child, authorize their participation in the Vacation Bible School activity organized by New Life Iglesia de Dios Pentecostal M.I.
I understand that all reasonable precautions will be taken to ensure the safety of theparticipants. However, in the event of a medical emergency, I authorize the leaders of the event to seek medical treatment for my child. I also authorize any medical provider to administer necessary treatment.
I acknowledge and accept that I, as the parent or legal guardian, will be responsible for any and all medical expenses resulting from any accident, injury, or emergency involving my child during this activity.
Furthermore, I release the organization, its leaders, volunteers, and affiliates from any legal liability for injuries, damages, or losses incurred during this event.
